This text briefly introduces visitors to your main services.
Bridge
Bridge follows the Assertive Community Treatment (ACT) model. The ACT model is a “team approach” (case managers, therapists, nurses, psychiatrist, peer support specialists, and budget specialist with Representative Payee Services) meeting the client’s where they are, often referred to as “hospital without walls”. Individuals on the Bridge team receive services in their homes, where they work and in other setting in the community where problems occur or where support is needed most.
