The Jail Transition Program (JTP) was created in 2016 to provide care for inmates in Utah County returning to society after incarceration. The JTP filled a gap in services within our community to address the high recidivism rates for individuals involved in the criminal justice system. Funding from the Justice Reinvestment Initiative (JRI) provided the means to expand our jail services and address the mental health needs of these individuals and enable them to make a successful transition from jail back into the community.

Who is Eligible?
JTP services are available to:
Individuals that are currently involved in the criminal justice system, have a mental illness and must be Medicaid eligible or are unfunded.
Intakes
Walk-in Times (No appointment needed)
Monday/Tuesday/Wednesday/Thursday/Friday 1:00 to 3:00 pm
We also offer morning walk in times for active Medicaid clients on:
Tuesday/Wednesday/Thursday from 9:00 am to 11:00 am
**Arrive early as Intake slots get filled fast on a first-come, first-serve basis.
Jail Transition Program (JTP) Services
Examples of services that we can assist clients with:
- Medication Management
- Therapy (Individual and Group)
- Case Management services (budgeting, applying for work, maintaining probation requirements, setting and keeping appointments)
- Assess for Substance Use Treatment and referrals
- Vocational Rehabilitation/Department of Workforce Services (Food stamps, Medicaid application, and General Assistance)
- Clothing Vouchers
- Food stamps/ PCN insurance/ Medicaid applications (as applicable)
- Assistance with housing applications
