Bring in your Medicaid card if you have one.
FOR THOSE WHO DO NOT HAVE MEDICAID, funding from grants and contracts may be available for specific services. Please call our clinic for more information.
Getting Started in Services
Getting started at Wasatch Behavioral Health is a friendly and simple process, starting with an intake appointment. The Westpark Family Clinic offers walk-in intake times for new clients each week so that you don’t have to wait for an appointment.
Please call and schedule an Intake Appointment if you would like more than one person in your family to be seen at the same time.
Walk-in intakes are held every Monday, Tuesday, and Thursday at 7:30 am (morning intake) and 11:30 am (afternoon intake).
How to Plan for your intake appointment
Intakes are on a first-come, first-served basis. Due to limited space, we encourage new clients to check in early.
Plan to stay for 2 to 2-1/2 hours. Arrange for childcare for your children at home. We are unable to provide childcare, and you will want to be able to focus on your appointment.
You and your therapist will work together to identify needs and goals and develop your personal Recovery Plan.
What to Bring to your Intake Appointment
- A driver’s license or photo ID. If you don’t have one, bring other identifying information such as: birth certificates, immunization records, social security card, and bank-card.
- Current Medicaid card(s). Bring a current Medicaid card with you to every appointment.
- Legal guardianship or custody papers, when applicable.
- A list of current medications.
