South Provo Clinic

The South Provo Clinic is located on the South Provo Campus in Provo. It is a program that focuses on adults with mental illness who may need more assistance in the community accessing resources and managing day-to-day functions such as shopping, arranging medical, dental and eye appointments etc. We offer therapy, psychiatric care with nursing services and medication management along with a heavy case management model. We also work closely with the housing authorities in the area and we have a specific focus on assisting clients in housing needs.

Getting Started in Services

If you are a current client at Wasatch Behavioral Health (WBH), talk to your therapist about your housing needs. They will discuss the referral process with you.

SUPPORTED HOUSING SERVICES

REQUIREMENTS & ELIGIBILITY FOR SUPPORTED HOUSING SERVICES
  • Must be a Utah County Resident
  • Must have Medicaid
  • Completion of a Mental Health Assessment to determine your needs meet this level of care
  • must be actively involved in treatment/services at WBH

SUPPORTED HOUSING PROGRAMS

Alpine House– 156 South 300 West, Provo, Utah
The United Way of Utah County provides supportive residential services with live-in house parents. Supervision is provided throughout the week, including holidays. Individuals must have activities away from Alpine House daily between 9:00 a.m. to 3:00 p.m.

Supported Residential Treatment (SRT)– 911 South 950 West, Payson, Utah
This facility provides independent living apartments with supervision from on-site house parents. Residents have the opportunity to practice skills of daily living, self-care, symptom management, and other structured care for a stable community life. 

Payson Independent Living– 911 South 950 West, Payson, Utah
These are independent living apartments with minimal supervision from on-site house parents. Residents must sign a one-year lease.

Mapleview Apartments– 1193 East 300 North, Provo, Utah
This is a complex of independent living apartments managed by Provo City Housing. Residents must sign a one-year lease.

Yarrow Apartments– 210 East 300 South, Springville, Utah
This is a complex of independent living apartments owned by the Housing Authority of Utah County. Residents must sign a one-year lease.
